Interim steps for psychological and neuropsychological testing form

This article is for behavioral health hospitals and facilities, child psychiatrists, clinical psychologists, community mental health centers, opioid treatment centers, psychiatrists, psychiatrist/neurologists, neurologists, and pediatricians.

As you know, new codes for psychological and neuropsychological testing took effect on January 1. The Mass Collaborative is developing a new prior authorization form that will include the new codes and remove codes that are no longer valid.

Until that form is available, please:

  • Use space on the current prior authorization form or an additional sheet to write in the codes you are requesting and the number of units you anticipate using. It is especially important to highlight the number of units for the testing evaluation codes since only these require authorization.
  • Provide additional information or clarification that you feel is not captured on the form. You may do this at any time you feel additional information would help us make our determination.

Our Psychological and neuropsychological assessment supplemental form will no longer include the prior authorization form. You can download the prior authorization form from the Mass Collaborative. We will provide a link to it from our website.
