Keep preventive care a priority for your patients

The COVID-19 pandemic has caused many people to defer preventive care. Identify which of your patients have delayed routine services, and please follow up with them to make sure they get scheduled now. To help, we’re sending wellness reminders to certain members in August.

Who will receive reminders?

All members with asthma, coronary artery disease, or diabetes who have gaps in their care will receive letters encouraging them to stay on track with managing their health.

  • This includes members who belong to our Medicare Advantage, commercial HMO and PPO, and Federal Employee Program (FEP) health plans.
  • The letters will tell members if they have not received critical health services (based on our claims data).

Asthma


Ages 5+ Medication to control asthma:
  • Steroid inhaler or another controller medication

Coronary artery disease


  • Male ages 21-75
  • Female ages 40-75
  • Statin use

Diabetes


Ages 18+
  • HbA1c
  • Eye exam
  • Urine test for kidney function
  • Statin use (age 40-75)

Preventive screening reminders

Eligible members may also receive mail, email, or text reminders about the importance of getting a:

  • Cervical cancer screening (members over age 19)
  • Colorectal cancer screening (members over age 51)
  • Dental visit
  • Mammography (members over age 52)
  • Routine health checkup

You can support the health and wellness of your patients with these conditions by reinforcing these messages to them, scheduling preventive care appointments, and discussing at-home screening options when appropriate.
