Kidney disease support program for commercial members through Cricket Health

In 2022, we will partner with Cricket Health, an independent company, to complement the care that our commercial* members living with chronic kidney disease (CKD) currently receive.

On March 2, 2022, Cricket Health will begin engaging a small portion of our commercial members who have stage 3b chronic kidney disease or higher to support them in managing their disease. 

Cricket Health will provide an extra layer of comprehensive, personalized support designed to enhance the quality of life for this commercial population, and will be delivered at no additional cost to you or the patient.

How Cricket Health will support your team:

Cricket Health’s care team works together with a member’s treating providers. They don't replace them.

Cricket Health will invite eligible members to participate in the program. Once enrolled, members can interact with nurses, pharmacists, social workers, dietitians, and trained peer mentors in real-time through the MyCricketTM digital platform. 

Unlimited access to Cricket’s multidisciplinary team provides each member with the individual support they need to stay on top of their health care, including:

  • Encouraging them to adhere to their care plan
  • Education about the importance of diet and nutrition, medication management, home modality options, the transplant process, and medical management of kidney disease
  • Remote monitoring
  • Access to a community of others who can offer real-life support on a personal level because they’ve gone through similar experiences

Resources Cricket Health will provide to you

Cricket Health will send participating providers a welcome kit that will include:

  • An introductory letter and email
  • Provider FAQs
  • A brochure for members participating in the program that explains how Cricket Health will work with their doctor to offer support between appointments and how to activate their MyCricket account

Each month Cricket Health will also send a report to nephrology practices indicating which patients are eligible for the program, who are enrolled, and who have left the program. Please note that Blue Cross is selecting the members to participate in this program at this time.

Like you, we want to help your patients enjoy life without the constant reminder of living with chronic kidney disease. Participation in the program is voluntary for your patients. We encourage you to tell them about the potential benefits, including:

  • Support in implementing your patient care plan
  • Care coordination between providers
  • Resources to answer patient questions
  • Help guiding patients to community resources and peer groups
  • Education on renal replacement therapy
