New in-home program for complex Medicare Advantage members

For providers caring for our Medicare Advantage members

We are collaborating with Accompany Health, an independent provider group that offers home-based services. They’ll deliver care to eligible Medicare Advantage members who may have complex needs due to multiple chronic medical and mental health conditions.

This in-home program provides eligible members with coordinated primary, mental health, and social care support at no additional cost to the member. The program complements members’ existing primary care provider relationships.

How your patients will be supported

Accompany Health offers complete in-home care to your patients (our members) and brings in specialists via virtual visits. Each member receives a comprehensive initial visit, regular proactive follow-ups, and is engaged daily during acute events. Accompany Health offers eligible members:

  • 24/7 in-person and virtual urgent care
  • Social support and community resource navigation through community health workers who will accompany patients to their doctor visits (when appropriate)
  • Comprehensive mental health services
  • Pharmacy support for medication adherence
  • Patient advocacy and care coordination
  • Health coaching
  • Assistance with food programs, safe housing, and picking up medications    

How Accompany Health will work closely with you 

Accompany Health will follow your lead and expand upon the exceptional care our members are already receiving from you. The member will have access to a “Care Pod.” This is an extensive care team that includes a nurse practitioner or physician assistant, and a community health worker who will work together to address psychiatric needs, chronic conditions, medication needs, and urgent care.

Accompany Health’s care team will keep you informed by:

  • Exchanging clinical notes on member-specific issues
  • Calling your office to discuss time-sensitive member issues, with the member’s consent
  • Holding in-person introductory meetings with your office, if requested
  • Contacting you immediately if the member has acute changes

Members can continue to see you in your office as often as needed.
