Steps we’re taking to help our members with diabetes

This article is for primary care providers and endocrinologists caring for our members with diabetes

Starting in September, and through the end of 2022, we’re testing a new collaboration with CVS Caremark®' to help close possible gaps in care for our members with diabetes. The goal is to help our members get recommended diabetic screenings and care for optimal health. Because we recognize the importance of the patient-provider relationship, we’ll steer members back to you—their PCPs and treating clinicians—whenever possible.

What is the collaboration?

We’ve identified commercial and Medicare Advantage members who may not have received the screenings or diabetic care listed below, based on claims data through June 30, 2022. We’re working with CVS Pharmacy®' and its care concierge service to help our members get the care they need.

CVS Caremark, an independent company, is working on our behalf to pilot this program for our members.

What will the CVS care concierge team do?

From September through December 2022, CVS’ team of care concierges will:

  • Reach out to identified members by phone to offer education and coordinate care for any of the necessary services below
  • Help schedule a visit with the member’s PCP, if they have one, to get the recommended care

Note: If the member does not have a PCP, our Member Service team can help them find one. Or, CVS’ care concierge will help the member schedule an appointment at one of their 32 CVS HealthHUB™ or MinuteClinic locations in Massachusetts.

CVS offers appointments via telehealth and in-person, and can conduct all of the services listed below, including tests or collecting lab samples.

5-Star quality rating measures for delivering recommended care to people with diabetes.

What communication takes place with the members’ care team?

If the member receives care through the HealthHUB or MinuteClinic, CVS will provide an after-visit summary through connected EHR systems. They use CareEverywhere for electronic sharing, though fax is also available, and the member would need to consent to sharing results with you.
