Support for Medicare Advantage patients with chronic kidney disease

This article is for primary care providers, endocrinologists, and nephrologists caring for our Medicare Advantage members

We hope you’ll recommend the Square Knot Health program to your Blue Cross Medicare Advantage patients with chronic kidney disease (Stage 3B, 4, 5, and end stage kidney disease). Since January 1, 2022, we’ve partnered with Square Knot Health, an independent vendor that offers personalized phone and video support to our Medicare Advantage members at no additional cost.

Square Knot Health has a team of health navigators who have personal experience with kidney disease or kidney transplant. These advisors work directly with participating members to educate and support them throughout their journey. 

The Square Knot Health program

While our Blue Cross care management team identifies and engages eligible patients for the Square Knot Health program, your recommendation is valuable to your Blue Cross Medicare Advantage patients*. Voluntary participation in the program offers many potential benefits, including:

  • Resources, education, and support for coping with the challenges of chronic kidney disease and the treatment care plan
  • Early planning and assistance for kidney transplant prior to the need for dialysis. Transplantation is the treatment of choice for kidney failure due to lower morbidity and mortality and improved quality and length of life.
  • Assistance overcoming barriers that may prevent listing for kidney transplant.

*Members age 84 and older or members with any of the following comorbidities are excluded from the program: Alzheimer’s and other significant dementias, some chemotherapies, and members on home oxygen and ventricular assist devices.

  • Be sure to check off the box indicating that the member belongs to Medicare Advantage in the form's Member and Provider Information section.
  • Please add a note to the Reason for Referral section that this request is designated for chronic kidney disease through the Square Knot program.
