May 15, 2024 is an important deadline for the calendar year 2023 behavioral health hospital incentive program. You must submit your facility’s information and data for the reporting measure (about your quality improvement initiative) by this deadline to be eligible for an incentive.
What information and data does your facility need to provide?
Please use our template to submit information on your quality improvement initiative (see instructions for downloading this below). Remember, you are submitting data for the performance period of January 1, 2023 through December 31, 2023.
