Improve SUD recovery with the critical 7-day follow-up

Connecting members to a follow-up appointment within seven days of discharge from high-intensity treatment for a substance use disorder (SUD) is critical for their recovery. It also improves a key HEDIS®′ quality measure. High-intensity treatment is defined as an acute inpatient hospitalization, residential treatment stay, or a withdrawal management visit.

About the HEDIS quality measure

Follow-Up after High-Intensity Treatment for Substance Use Disorder (FUI) tracks how many commercial members receive a follow-up visit within seven days of discharge.

  • Better outcomes: Timely follow-up care is proven to reduce relapse rates, prevent hospital readmissions, and increase a member's adherence to their discharge treatment plan.
What you can do

For discharging facilities:

  • Act early: Begin discharge planning as soon as treatment starts.
  • Schedule appointments before the member is discharged: Secure a follow-up appointment for the member within seven days of them being discharged.
  • Educate: Clearly explain the importance of attending the follow-up visit to the member and their caregivers. On the discharge summary, include appointment instructions, medication specifications, and a plan for crisis or relapse prevention.

For outpatient providers:

  • Be available: Offer appointment times within seven days for members recently discharged from SUD treatment. This could be in-person or through telehealth, including a telephone visit.
  • Bill promptly: Submit claims (with a substance use disorder diagnosis) and encounter data in a timely manner so the follow-up visit is recorded.
  • Collaborate: Coordinate care with a member's other treating providers to ensure a seamless transition.

When members are seen promptly after discharge, their path to recovery is stronger. Your partnership is essential to making this happen.
