Here are some upcoming tasks you’ll need to complete to become eligible for an incentive as part of the annual skilled nursing facility (SNF) incentive program.
Reminder: Transitional care units are not eligible to participate in the SNF incentive program.

Provider Central The person submitting your facility’s data for the incentive program must be registered for Provider Central to log in and report the information. Learn how to register for Provider Central or add users to your existing Provider Central account. |

Our templates provide you with questions you’ll need to answer to report your facility’s performance on each measure. You can download these now (available in Word or PDF format) as you work on the measures, so you are ready to submit your responses in the online reporting tool starting June 1.
Please answer each question as completely as possible for full credit. We’ve provided notes or prompts to help guide you. See example below.
