SNFs: Incentive reporting is due June 15

This article is for administrators, directors of nursing, and quality improvement directors at skilled nursing facilities caring for our members.
(Transitional care units are not included in the incentive program.)

Our online reporting tool is now available for the 2026 Skilled Nursing Facility Incentive Program. Submit your information by June 15, 2026.

To be eligible for an incentive
  • Register for Provider Central.
  • Answer questions and share data using the online reporting tool (above) for the Transitions of Care measure.
Reporting best practices
  • Have your NPI ready to access the reporting tool.
  • Gather your information before you start.
  • Use the Transitions of Care template. Fill it out in advance to easily copy and paste information into the reporting tool.
  • Answer each question completely for full credit.
  • If you are part of a larger organization with system-wide protocols, policies, and procedures, your responses should be specific to your facility.
Tips for using the reporting tool
  • Copy and paste information from the template to save time. There are no time limits set on our links, but your browser setup may have a time limit.
  • Provide the contact information for the person who should receive the 2026 SNF Incentive Program outcome letter (usually the administrator or managed care contact).
  • Verify your email address is spelled correctly to receive submission confirmation.
  • Click “Submit.”
  • If you need to stop, the survey will automatically save. Our Alchemer survey tool will email you a link to continue.
  • Save a copy of your submission. You’ll get an email with a PDF of your responses. Keep it for your records.​
