Use the Member ID when submitting dental claims

Due to the Health Insurance Portability and Accountability Act (HIPAA), we do not accept Social Security numbers on claims.  Although the form indicates "SSN or ID#", remember to use only the member’s Blue Cross Blue Shield of Massachusetts Dental Blue Member ID.

If your office still submits paper claims, only use the most current 2012 ADA claim form:

Also, please remember to use the correct field for the NPI number.


  • Login and go to Office Resources>Administrative & Policy Guidelines >Provider Manuals>Dental Blue Book. (section 5)
  • Call the Dental Information Center 1-800-882-1178 and select menu option 3
