Documenting weight, nutrition, and activity counseling for children and adults

The Health Care Effectiveness and Data Information Set (HEDIS) measures the percentage of your patients who have had face-to-face outpatient visits with their physician during the measurement year. Here are the requirements for children, adolescents, and adults.

For this age group HEDIS requires
Children and adolescents, ages 3-17 that when counseling, you note in the patient’s medical record once during the measurement year:
  • their body mass index (BMI) percentile either on a graph or documented as a percentile.
  • that you have discussed or counseled for nutrition and physical activity. The entry must list the date and details of the counseling conversation.
Adults, ages 18-74
  • that you document once during the measurement year the patient’s height, weight and BMI.
  • BMI must be plotted on a graph or documented as a percentile for individuals ages 18-20.
