We want to remind you that as of January 1, 2024, we have retired the InterQual®′ criteria for neuropsychological testing. Now all medical necessity criteria for both neuropsychological and psychological testing are in our medical policy 151: Neuropsychological and Psychological Testing. We notified you of this change in our October 2023 medical policy announcements.
Why did we do this?
We reorganized the medical necessity criteria and streamlined the information into a singular document to help you quickly and easily understand how we make coverage decisions. Medical policies are reviewed at least annually to ensure they reflect the most updated clinical references, including InterQual Level of Care Criteria for Behavioral Health.
How to request authorization for testing
Prior authorization is still required for commercial HMO and POS members. There are no changes to the way you request prior authorization. Please continue to use one of these methods:
- Submit your request electronically using Authorization Manager
- Use the Psychological and Neuropsychological Assessment Form and fax it to us
- Call us at 1-800-524-4010
