This article is for primary care providers and endocrinologists caring for our members with diabetes
Starting in September, and through the end of 2022, we’re testing a new collaboration with CVS Caremark®' to help close possible gaps in care for our members with diabetes. The goal is to help our members get recommended diabetic screenings and care for optimal health. Because we recognize the importance of the patient-provider relationship, we’ll steer members back to you—their PCPs and treating clinicians—whenever possible.
We’ve identified commercial and Medicare Advantage members who may not have received the screenings or diabetic care listed below, based on claims data through June 30, 2022. We’re working with CVS Pharmacy®' and its care concierge service to help our members get the care they need.

Note: If the member does not have a PCP, our Member Service team can help them find one. Or, CVS’ care concierge will help the member schedule an appointment at one of their 32 CVS HealthHUB™ or MinuteClinic locations in Massachusetts.
CVS offers appointments via telehealth and in-person, and can conduct all of the services listed below, including tests or collecting lab samples.

If the member receives care through the HealthHUB or MinuteClinic, CVS will provide an after-visit summary through connected EHR systems. They use CareEverywhere for electronic sharing, though fax is also available, and the member would need to consent to sharing results with you.
