Submit required clinical to avoid delays in auth processing

This article is for skilled nursing facilities, transitional care units, long-term acute care hospitals, rehabilitation hospitals, and acute care hospitals caring for our members

It’s important to submit all required clinical information when requesting authorization (initial precertification) for skilled nursing, transitional care unit, long-term acute care hospital, rehabilitation hospital, and acute care hospital services.

Missing or incomplete information requires us to follow up with your office. This results in delays to our medical necessity review, our response to your request, and ultimately the patient’s transition to the next level of care.

What to submit to ensure timely processing

To facilitate processing and avoid delays in determinations, you can either submit the authorization request via Authorization Manager, or complete and submit the Initial Precertification Form for SNF/Rehab/LTCH.

If you submit by Note
Authorization Manager
  • You do not need to fill in the member and provider demographic information in Sections A and B as you have already completed these fields in Authorization Manager.
  • Include the name of the acute attending physician in your submitted clinical documentation.
Completing the Initial Precertification Form for SNF/Rehab/LTCH
  • Complete required Sections C through I on the form.
  • Include the name of the acute attending physician in section B on the form.
  • Upload the completed form with pertinent additional clinical information.
