If you need to make changes to an existing authorization, such as the date of service, codes, or units, we’ve created a new form for you to do so. Once you’ve completed the form with your information and the requested updates, fax it to us at the number on the form. Note: The fax number you use will depend on the type of service being rendered and the member’s plan.
