Our Member Appeals and Grievances area has received an influx of faxes and appeal forms incorrectly submitted to them by providers. We’d like to remind you to direct your appeals to the attention of our provider appeals teams so that we can quickly process them for you.
To make it easier for dentists, we’ve created a new dental-specific form you can use to submit claim appeals to us.
Please use our Request for Dental Claim Review form to request a claim review. Be sure to attach any relevant support documentation (such as X-rays, charts, etc.).

Note: If you’re a medical provider who wants to correct a previously submitted claim, remember that you can submit a replacement claim, not an appeal.
