Send your appeals to the correct area for faster processing

Our Member Appeals and Grievances area has received an influx of faxes and appeal forms incorrectly submitted to them by providers. We’d like to remind you to direct your appeals to the attention of our provider appeals teams so that we can quickly process them for you.

Dentists: New form for claim appeals

To make it easier for dentists, we’ve created a new dental-specific form you can use to submit claim appeals to us.

Please use our Request for Dental Claim Review form to request a claim review. Be sure to attach any relevant support documentation (such as X-rays, charts, etc.).

Mail completed forms to the address listed on the form.

To help us process your appeals faster and avoid re-work, please be sure to:

  • Review our Administrative Reviews and Appeals information on Provider Central to use the correct forms and find the correct address or fax number.
  • Replace old forms you’ve saved to your desktop with the new form. You can find our forms in the forms library on Provider Central.
  • Update fax numbers or mailing addresses that you use, if necessary, so you’re using correct contact information.

Note: If you’re a medical provider who wants to correct a previously submitted claim, remember that you can submit a replacement claim, not an appeal.
