As you are aware, members may have the right to receive in-network continuity of care from their health care provider for a period of time after their health care provider leaves the Blue Cross Blue Shield of Massachusetts network.  

The Centers for Medicare & Medicaid Services (CMS) requires that continuity of care apply to Medicare Advantage members.

What this means

• Medicare Advantage members who join Blue Cross Blue Shield of Massachusetts during an active course of treatment are eligible for continuity of care for the first 90 days.  

• We updated our Continuity/Transition of Care Request form so it can be used for Medicare Advantage members applying for in-network continuity of care.

What you need to do

Please notify us of any new Medicare Advantage members who are in an active course of treatment by completing the Continuity/Transition of Care Request form.
