Skilled nursing facility incentive program: Deadlines, reminders

This article is for administrators, directors of nursing, and quality improvement directors at skilled nursing facilities caring for our members.

Here are some upcoming tasks you’ll need to complete to become eligible for an incentive as part of the annual skilled nursing facility (SNF) incentive program.

Reminder: Transitional care units are not eligible to participate in the SNF incentive program.

By this deadline You must
May 1, 2021 Register for Provider Central
The person submitting your facility’s data for the incentive program must be registered for Provider Central to log in and report the information. Learn how to register for Provider Central or add users to your existing Provider Central account.
June 15, 2021 Submit your facility’s data for both program measures:
  • Medication safety
  • Transitions of care
The online reporting tool will be available on our website starting June 1, 2021. We’ll send you an email when you can start using it. We will only accept submissions we receive through our reporting tool on Provider Central. Emailed or mailed submissions will not be accepted.

How can your facility get ready to report performance?

Our templates provide you with questions you’ll need to answer to report your facility’s performance on each measure. You can download these now (available in Word or PDF format) as you work on the measures, so you are ready to submit your responses in the online reporting tool starting June 1.

Reminders for reporting

  • If you are part of a larger organization with system-wide protocols, policies, and procedures, your reporting responses should still be specific to your facility.
  • When answering each question, please include a description on how the COVID-19 pandemic has impacted your processes, if applicable.

Please answer each question as completely as possible for full credit. We’ve provided notes or prompts to help guide you. See example below.

For the transitions of care responses:

  • Census tracking is for all of your patients, not just Blue Cross Blue Shield of Massachusetts members.

For the medication safety responses:

  • On question 12, if you answer “yes” to having treatment protocols for any condition, please give a comprehensive response for each condition so we better understand what processes you have in place throughout the year. See example below.
