Skilled nursing facility incentive program reporting due June 15

This article is for administrators, directors of nursing, and directors of quality improvement at skilled nursing facilities.

The online reporting tool for the 2019 skilled nursing facility incentive program is now available. Please use the tool to submit your information to us by the June 15, 2019 deadline.

Once you’ve entered your data into the reporting tool and have submitted it to us, you will receive an email confirmation and a copy of the information you submitted. We urge you to save this email in case you are asked to confirm your submissions to us.

Important tips for submitting your data
Before you enter your data into the online reporting tool:

  • Have your NPI ready. You need to enter it.
  • Gather information you’ve been collecting for your submission. Templates were available on our site to help you collect data during the reporting period. 

When using the reporting link (tool):

  • If you start entering information into the tool, but need to stop, the survey will automatically save your responses for later.
  • Cut and paste information (from the templates you filled out during your data collection) into the tool to go faster. There are no time limits set on our links, but your browser setup may have a time limit.
  • Verify that your email address is spelled correctly. If it is not, you will not receive the email confirmation.
  • Click "Submit" to submit your responses.
