Submit your facility’s data for the program measure:
The online reporting tool will be available on our website starting June 1, 2022. We’ll send email when you can start using it. You’ll need to enter the NPI of the facility that you are reporting for in order to access the tool.
We will only accept submissions we receive through our reporting tool on Provider Central. Emailed or mailed submissions will not be accepted.
Our template provides questions you’ll need to answer to report your facility’s performance on each measure. You can download these now (available in Word or PDF format) as you work on the measures, so you are ready to submit your responses in the online reporting tool starting June 1.
Download templates and get additional information on our SNF incentive program page. Log in and go to Quality & Performance>Performance Programs>SNF Incentive Program.
