SNFs: Are you ready to qualify to earn an incentive?

Learn how to register for Provider Central or add users to your existing Provider Central account.

We post information and email reminders about the incentive program to registered Provider Central users, like the one we'll send on June 1 when the reporting period opens. For this reason, it’s important that you and any staff at your facility are registered for our website.

June 1, 2024 Gather information you need for reporting. The online reporting tool is ready for submissions.

On June 1, we’ll send an email to our registered Provider Central users with a link to an online tool to report your information to us. To access the tool, you’ll need to enter the NPI of the facility on which you are reporting. You’ll have until the June 15 deadline to submit your Transitions of Care information to us.

June 15, 2024 This is the last day to submit your facility’s data for the Transitions of Care measure

We will only accept submissions we receive through the reporting tool and by the June 15 deadline. We cannot accept emailed or mailed submissions.

Important reminders

  • We’ll email the 2024 SNF Incentive Program outcome letter to the person listed as the contact in the submission tool. This person should be the administrator or managed care contact. We need this person’s name, title, and email address. Please make sure you list the correct email address on the 2024 SNF Incentive Program submission tool.
  • If you are part of a larger organization with system-wide protocols, policies, and procedures, your reporting responses should still be specific to your facility.
  • Please answer each question as completely as possible for full credit. We’ve provided notes or prompts to help guide you.

Use our Transition of Care template to report answers to questions about your facility for this measure of the incentive program. Log in and download the template now (available in Word or PDF format) as you work on the measure, so you are ready to submit your responses in the online reporting tool starting June 1.
