SNFs: It’s incentive program reporting time. June 15 is due date.

This article is for administrators, directors of nursing, and quality improvement directors at skilled nursing facilities caring for our members.
(Transitional care units are not included in the incentive program.)

Our online reporting tool is now available for the 2024 Skilled Nursing Facility Incentive Program! Please use it to submit information to us by no later than June 15, 2024.

What do you need to do to be eligible to earn an incentive?

  • Register for Provider Central (you and anyone else in your organization who needs to receive news and updates about this program).
  • Answer questions and share data through the online reporting tool (above) for the Transitions of Care measure.
  • Complete our 31-minute educational training: https://www.brainshark.com/bcbsma/snf24
    Note: Completing the training was due by May 31. However, we’re offering additional time if you haven’t yet completed it. You have until June 15, 2024 to watch the full training. Anyone in your facility who takes the training (all job roles) will help you qualify for an incentive, as long as you've completed the Transition of Care measure reporting.  

Reporting best practices

Here are some tips:

  • Have your NPI ready. You need it to get into the tool.
  • Gather information you’ve been collecting for your submission. Use the Transitions of Care template. If you fill out the template in advance, you can easily copy and paste information into the reporting tool.

When using the reporting link (tool):

  • Copy and paste information from the template into the tool to go faster. There are no time limits set on our links, but your browser setup may have a time limit.
  • Verify that your email address is spelled correctly to ensure you receive the confirmation of your submission.
  • Click “Submit.”
  • If you start entering information into the tool but need to stop, the survey will automatically save your responses. The company we use for the survey tool, Alchemer, will send you an email when you save your submission. Click the link in the email to pick up where you left off.

Once you submit the survey, you’ll get an email that contains a PDF of your responses. We encourage you to save a copy of your submission in case there’s a technical issue and you need to substantiate what you submitted.
