Attention: Skilled nursing facility administrators and quality improvement directors
Here are some upcoming tasks you’ll need to complete to become eligible for an incentive for the annual skilled nursing facility (SNF) incentive program.
Reminder: Transitional care units are not eligible to participate in the SNF incentive program.

|May 1, 2019||Register for Provider Central
The person submitting your facility’s data for the incentive program must be registered for Provider Central to log in and report the information.

|June 15, 2019||Submit your facility’s data for all of the program measures:
The online reporting tool will be available on our website starting June 1, 2018. We’ll send you an email when you can start using them.
How can your facility get ready to report performance?
Our templates provide you with questions you’ll need to answer to report your facility’s performance on each measure. You can download these now (available in Word or PDF format) as you work on the measures, so you are ready to submit your responses in the online reporting tool starting June 1.
