We’re requesting postpartum care information

We want to make sure our members—your patients—receive postpartum care.  This should happen within the recommended 21-56 day window, which follows American College of Obstetricians and Gynecologists (ACOG) guidelines and HEDIS specifications.

Starting in late July, we are reaching out monthly to OB/GYN groups and other practices to request information about our members’ postpartum care. We’ll contact you by phone to confirm your fax number and then fax you a list of patients. You can enter the information right on the form and return it to us by fax.

We appreciate you taking the time to review this information and share data with us.

Here’s the information we need:

  • For patients who have recently delivered:
    • The postpartum visit date. (If this hasn’t happened yet or if it was cancelled, please schedule the visit to meet the ACOG timeframe and guideline.)
  • For patients who are pregnant and our records don’t indicate the delivery date:
    • Expected delivery date (EDD)/Expected date of confinement (EDC)
    • Actual delivery date
    • Date the postpartum visit is or was scheduled
    • Name of provider now seeing the patient (if transferred) and their phone number, if known
