Don’t miss out on potential incentive dollars for your facility! Deadlines for the 2026 Skilled Nursing Facility Incentive Program are fast approaching, and a few key actions now will help your facility prepare to report.
To be eligible for incentive payments, facilities must complete specific registration and reporting tasks.
It is critical that your facility's information is up-to-date. We use this information to send important reminders, including one on June 1 with the link to the reporting tool.
- Log in to Provider Central.
- Go to My Profile to verify your email and phone number.
- Go to My Organizations to ensure your account is affiliated with the correct facility NPI and provider number.
- If your affiliation is incorrect, click Add an organization to update it.
You will need the facility’s NPI and tax ID number (TIN).
For help deleting an organization (after you’ve added a new organization), you may contact our Provider Central support team (see Technology section of Contacts & Sites).
| May 1, 2026: Register and verify. This is the deadline to register for Provider Central and ensure your profile is correct. If you aren't registered, go to bluecrossma.com/provider and click "Register." If you need help, view guides on our Learn more about Provider Central page. |

| June 1, 2026: Reporting begins. We will email registered users a link to the online tool to report your Transitions of Care measure data. |
|
| June 15, 2026: Reporting ends. This is the final day to submit your facility’s data. |
- Be prepared: Download the Transitions of Care template now to prepare your responses and review Program requirements. Log in and go to Quality & Performance>SNF Incentive Program.
- Be specific: Even if you are part of a larger organization with system-wide protocols, policies, and procedures, your answers should be specific to your facility.
- Be complete: Answer every question as thoroughly as possible to receive full credit.
- Submit online: We will only accept submissions through the official online reporting tool available on June 1. Emailed or mailed data will not be accepted.
- Get notified: We’ll email the 2026 SNF Incentive Program outcome letter to the person listed as the contact in the submission tool. This person should be the administrator or managed care contact. We need this person’s name, title, and email address. Please make sure you list the correct email address on the 2026 SNF Incentive Program submission tool.
