Replacement Claims

If you want to make a correction to a previously submitted 1500 or
UB-04 claim, submit a replacement claim (no supporting documentation needed).

Follow this process to:

  • Correct the date of service, diagnosis, procedure, or modifiers
  • Correct patient data (some exceptions1 apply)
  • Make changes to your original claim, plus add new charges for services not previously submitted.
  1. Do not submit a replacement claim to change the subscriber ID number. For other exceptions, refer to our Frequently Asked Questions.
