About our Home Infusion Therapy network

Home infusion therapy providers offer a range of safe, convenient, cost-effective infusion services to our members. Experienced home infusion nurses and pharmacists help our members get the immunoglobulins, enzyme replacements, antibiotics, and other infused medications they need, wherever they need them—in the home, at work, or in the setting of their choice.

How to refer members for home infusion therapy

Step Go to Find a Doctor & Estimate Costs and type Home Infusion Therapy into the search window

Step Call the HIT provider and make the referral
 
Step Fax or e-prescribe the prescription to the HIT provider

The HIT provider will then request authorization from us for the services.

Once you’ve received the referral from the member’s physician or clinician, you’ll need to:

Step Check member benefits and eligibility using our technologies
 
Step Review our medical and payment policies for information on diagnosis and medication coverage
 
Step Request prior authorization. Complete the Home Infusion Therapy Prior Authorization Form and fax it to 1-888-641-5355
